Was actually asked in a separate interview 4 years later for a different position. My answer was yes, and that sole accountability was something I had an opportunity to improve upon. I didn't get that job, and was told in the follow-up that I lacked "accountability". Go figure. Tip: Don't be realistic, just tell them what they want to hear. Acceptable response may have been, "No, I am a perfect human being and have zero flaws, both in my professional and personal life, thank you very much."
