How do you deal with difficult conversations with a co-worker?

If I were to have a difficult conversation with a coworker, it would usually be because there was a major error in a report or something of a similar nature. However, it's the overall team's fault because there should always be checks and balances when it comes to what is being produced. Therefore, the difficult conversation would be more focused on why the issue occurred and how we, as a team, can prevent it in the future.
