We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This position requires in-person attendance at 5 different Brooklyn clinics with travel in support of the CCBHC Project.

Certified Community Behavioral Health Clinic (CCBHC) Core Services includes:

• Crisis mental health services including 24-hour mobile crisis teams, emergency crisis intervention, and crisis stabilization; Screening, assessment, and diagnosis including risk assessment; Patient-Centered treatment planning or similar processes, including risk assessment and crisis planning; Outpatient mental health and substance use services; Outpatient clinic primary care screening and monitoring of key health indicators and health risk; Targeted case management; Psychiatric rehabilitation services; Peer support, counselor services, and family support services; and, Intensive community-based mental health care for members of the armed forces & veterans.

SUMMARY: A Certified Community Behavioral Health Clinic (CCBHC) The Psychiatric Rehabilitative Specialist (PRS) provides recovery-oriented, community-based mental health services to individuals with serious mental illness/ substance abuse. In this role you will support clients in developing skills, accessing resources, and achieving personal goals related to independent living, employment, education, social functioning, and overall wellness.Your contributions will directly support our mission of enhancing community well-being.

JOB DESCRIPTION:

• Provide psychiatric rehabilitation services focused on skill-building, recovery, and community integration to children, youth, adults and their families.
• Assist children, youth, adults and their families in developing and achieving individualized person centered recovery goals.
• Assist, support and reinforce daily living skills, including self-care, personal hygiene, budgeting, medication management, time management and more to children, youth, adults and their families.
• Support the development of personal autonomy, self-advocacy, wellness, recovery and independent decision-making.
• Support children, youth, adults and their families in identifying their rights, needs, and preferences.
• Assist children, youth, adults and their families with dietary education which in compasses but not limited to healthy meal planning, shopping and preparation.
• Assist children, youth, adults and their families in accessing their community such as transportation, route planning, time awareness and identifying transportation resources to increase independence.
• Assist with community integration activities, including building natural supports and social networks.
• Assist with increasing community awareness and community tenureship.
• Assist in facilitating effective communication and interactions with health care professionals.
• Assist children, youth, adults and their families in managing mental health /substance use symptoms, and medication routines in coordination with treatment teams.
• Provide psychoeducation related to mental health, recovery, wellness, and self-management.
• Support benefits navigation and basic financial management skills with practice and coaching activities.
• Assist individuals in connecting to the development of employment goals, including readiness for IPS activities.
• Assist individuals in connecting with school and achieving academic goals, including readiness for IPS activities.
• Observe and report changes in mental status, functioning, or risk, and communicate concerns to supervisors.
• Collaborate with licensed clinicians, psychiatrists, nurses, and care coordinators as part of the IOP multidisciplinary team.
• Support client participation and retention in IOP services.
• Provide crisis prevention support and assist with de-escalation strategies as needed, following IDCC protocols
• Participate in ongoing training, supervision, and professional development activities.
• Complete all assigned PRS training at onboarding and as required to maintain PRS skills.
• Collaborate with an interdisciplinary team which includes therapists, care coordinators, other certified peer specialists, mobile crisis clinicians, psychiatric nurse practitioners and psychiatrists to assist with an integrated person-centered goal planning.
• Communicates to the supervisor about the need for proper treatment and referral protocols with respect to child abuse, domestic violence, consultation with specialty clinics, and referral both within and outside of IDCC.
• Follow policies, procedures, and protocols to ensure compliance with CCBHC program requirements and regulations for substance abuse services.
• Complies with billing and reimbursement requirements, maximizing revenue potential while maintaining program integrity.
• Comply with all relevant regulations, accreditation standards, and reporting requirements.
• Participate in audits/reviews to ensure adherence to program requirements and standards.
• Complete agency training to improve cultural competency and trauma informedcare as they align to CCBHC and Agency specific mission.
• Perform other work-related duties as assigned.
• Available to travel/work evening and weekend hours as required by the agency.
• Work collaboratively with the CCBHC Interdisciplinary Team as well as external agencies and community stakeholders.
• Attend required management and staff meetings. Complete CCBHC and organizational training.

EXPERIENCE:

• 2 years direct experience with the development and provision of psychiatric rehabilitation, behavioral health related programming, and services for the special needs population with a multidisciplinary team approach.
• Knowledgeable about the psychosocial aspects of illness, health and recovery from a mental health and/or substance disorder struggle.
• Ability to work efficiently in a fast-paced environment.
• Proficient with word processing, electronic health records, and electronic communications.
• Well organized with strong time management skills.
• Able to work independently and as a member of a team.
• Excellent interpersonal skills, including being able to maintain a professional temperament with both clients and colleagues.
• Able to meet program standards in documenting in client records.
• Ability to work with challenging clients.
• Comfortable with facilitating direct community outreach and home visits.
• Comfortable and competency working with diverse communities in Brooklyn.
• Bilingual skill in a language other English (i.e. Spanish, Russian or Cantonese) is desired.

QUALIFICATIONS:

• At least a high school degree, BA degree preferred.
• Certified Psychiatric Rehabilitation Practitioner (CPRP) certification preferred: ability to obtain within two (2) years of hire.
