Job Summary

We are seeking a compassionate and dedicated licensed Long Term Care Social Worker to join our team at a prominent area nursing home. In this vital role, you will provide comprehensive social work services to residents, their families, and the care team. Responsibilities

• Conduct thorough patient assessments to identify social, emotional, and environmental needs of residents with a focus on geriatric care and dementia-related conditions such as Alzheimer’s disease.
• Develop individualized care plans in collaboration with healthcare teams, residents, and families to address psychosocial challenges and facilitate optimal discharge planning.
• Provide clinical counseling, including mental health support, behavioral therapy, crisis intervention, and end-of-life care discussions aligned with clinical ethical guidelines.
• Coordinate community resources such as hospice care, home & community care services, and specialized programs for disabilities or developmental conditions like autism or PTSD.
• Maintain accurate medical documentation and medical records in compliance with HIPAA regulations and social work practice standards.
• Facilitate group therapy sessions and individual counseling to support mental health wellness among residents from diverse cultural backgrounds.
• Engage in case management activities including intake procedures, discharge planning, and ongoing client communication to ensure seamless continuity of care.

• Wage range negotiable with years of experience.

Qualifications

• Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited institution; licensure or certification as required by state law.
• Proven experience working with geriatric patients in nursing homes or assisted living settings; inpatient or outpatient experience preferred.
• Knowledge of medical terminology, ICD coding systems (ICD-9/10), CPT coding, Medicare regulations, and electronic medical record (EMR) systems. Knowledge of PointClick Care electronic charting.
• Familiarity with hospice & palliative medicine principles, dementia care strategies—including Alzheimer’s—and behavioral health interventions such as cognitive behavioral therapy (CBT) and motivational interviewing.
• Strong understanding of social work practice standards, crisis management techniques, and cultural competency when working with individuals from diverse backgrounds.
• Ability to perform patient assessments, conduct diagnostic evaluations, and collaborate effectively within interdisciplinary teams.
• Excellent client communication skills coupled with compassionate listening abilities; adept at handling sensitive situations involving end-of-life discussions or mental health crises.
