This non-exempt (FLSA) position’s incumbent is the responsible liaison between Southside Behavioral Health (SBH) and all state mental health facilities, jails and court systems for discharge planning, assessment of services needed, coordination of service delivery programs, and clear, ongoing communication with individuals served, the SBH discharge planning team, the facility treatment team, their families, and assigned therapists and case managers. Incumbent represents SBH with effective, positive presentation skills and in a highly professional manner at all times. Incumbents are required to write individualized service budgets, negotiate contracts with licensed, and private providers. Incumbent will provide reporting on a regular basis on the use of funding and contracted services. Required to participate in regional utilization meetings. May require work outside of normal office hours including evening and weekend hours, may require travel to state and private hospitals across the state. Additionally this individual will be responsible for coordination of forensic assessments, restoration services and communication with all parties including legal council and court systems.

Responsibilities

• Serves as SBH’s liaison to state mental health facilities to coordinate discharge planning for individuals who are admitted to state hospital facilities by the court system (forensic clients) or correctional institutions.
• Follows state and regional guidelines and protocol for individuals in the state hospital system and has a working knowledge of the IDAPP manual and follows the manual in the creation of IDAPP plans, as needed for forensic clients.
• Works collaboratively with discharge planning teams, facility treatment providers, individuals, the court system, jail services, their families and all identified community providers to create a strength based, individualized discharge plan.
• Attends or coordinates treatment team meetings at state hospitals where individuals are identified at SBH clients or individuals needing transfer into SBH catchment area by other CSBs. Meetings should be attended by phone, virtually, or in person as needed. Follow-up should be completed within 24 business hours if unable to attend.
• Coordinates and provides restoration services to identified clients.
• Coordinates forensic assessments as required by the court with identified providers.
• Attends court to represent the client and SBH as needed and required.
• Tracks hospitalizations due to emergency prescreens and maintains the hospitalization log for all locations, creates monthly reports, creates weekly reports for the discharge planning team and ensures that documentation in the EHR is complete and timely.

Qualifications

• Bachelor’s degree in a human services field, with 1-2 years of experience of providing supportive services to individuals with serious mental (SMI), substance use disorders, intellectual disabilities or co-occurring disorders.
• Valid driver’s license and safe driving record, required.
• Qualified Mental Health Professional (QMHP) eligible.

Pay: $44,435.00 - $61,098.00 per year
