Salary dependent on education, licensure and experience
Location: Finger Lakes ACT (Geneva) M-F Day 8-4:30
Pass Days: Saturday/Sunday (rotational on-call required)
JOB DUTIES:
1. Serve as the Vocational Specialist - Serve as Vocational Specialist for the team, including, but not limited to the following: completing vocational assessments; coordinating with ACCESS VR; guiding clients in writing resumes; job coaching; linking with educational services; and creative job/small business development.
2. Provide therapeutic counseling in community setting to clients and their families as appropriate.
3. Assist clients in identifying, providing, and obtaining resources to enable them to continue to reside successfully in the community (case management, residential services, vocational services, transportation).
4. Assist clients in developing and maintain an Individual Service Plan
5. Maintain and document in assigned client treatment records
6. Provide substance abuse counseling, crisis intervention, client/family education, and assistance with self-administration of medication.
7. Provide on-call coverage for entire ACT census, on a rotational basis.
9. Provide crisis intervention.
10. Provide client/family education regarding mental illness and the recovery process.
11. Work closely with ACT multi-disciplinary team to ensure an ongoing dialogue regarding client*s progress/problems.
12. Complete other duties as assigned.
Minimum Qualifications:
Possession of a license and current registration as a Licensed Master Social Worker (LMSW) in New York State.
Must meet the Civil Service requirements for a HELPs appointment to the qualifying title.
Must have a valid Driver's License
