Job Overview
This position is for skilled and reliable Physical Therapists seeking freedom and flexibility as an independent contractor to take on referrals at an as-needed, though high-demand, basis. The ideal candidate does not need to consider this as a full-time role; in fact, many therapists in these positions can easily hold their own full or part-time positions elsewhere while still taking advantage of this job opportunity on the side! This role involves seeing as high or as low of a caseload desired-- there is no daily nor even weekly minimum quota-- of helping patients enhance their functional mobility within the comfort of their homes. This posting is currently for Solano County, but referrals are received from all counties of the Bay Area and this need can meet where you want to work, when you want to work. All experience ranges accepted, however if lacking in home care, we can provide guidance and training to the right candidate.

Pay-Per-Visit Rates
As an independent contractor, you would be compensated for the visit you complete (start of care, evaluation, follow-up, etc.) and track them on an invoice sheet. We do not pay an hourly rate, as you would not be limited as a W2 employee but rather liberated as a 1099 self-employed entity to make your own hours and take in as many or as little of a caseload as you'd like!

Duties

• Responsible for the evaluation, treatment, reassessment, and discharge of all referrals they accept.
• Promptly respond to referrals and collaborate with the multidisciplinary team to ensure continuity of care across acute care settings, clinics, and home environments.
• Write the plan of care to be followed by another PT and/or PTA if the evaluating PT does not continue visiting, conferencing as needed with PTA
• Communicate with the agency and referring provider to report status at evaluation, reassessment, discharge, and if any irregularities arise.
• Schedule their own visits to allow for complete autonomy and flexibility.
• Complete their invoice and submit within the contract's scheduled timeframe for efficient payroll organization.
• Complete all documentation within 24-48 hours of visit.
• Maintain accurate documentation of patient progress using EMR systems.
• Provide education to patients and their families regarding treatment options, home exercises, and discharge planning.

Requirements

• Valid Physical Therapist license in CA (PTLA considered upon interview)
• Graduate degree from an accredited physical therapy program by an approved and recognized organization
• Valid CA driver's license with a reliable vehicle and associated auto insurance
• Familiarity with EMR systems for efficient patient documentation.
• Excellent communication skills to effectively interact with patients, families, and healthcare professionals.
• Ability to work collaboratively within a team-oriented environment while demonstrating strong organizational skills.
• Previous experience with discharge planning and developing care plans is highly desirable.
