Pay: $48,000 - $50,000 with an additional $3,000-$4,500 in mileage reimbursement.

The Team Coordinator is responsible for the following:

Responsibilities

• Provide oversight for Individualized Supported Living (ISL) homes and follows Graystar, LLC standards and guidelines.
• Assist the Program Manager with oversight of documentation in the ISL homes, coordination of medical care and making sure all Department of Mental Health requirements are met.
• Help our Human Resource Manager to ensure that adequate staffing is provided for each home and occasionally fill in if needed.
• Planning and overseeing team building training for each home for the team leads and direct care staff. Also, overseeing the quarterly events for the individuals we provide services for and the staff.
• Assist in team leader training and shadowing as necessary.
• Help assist team leaders with staffing needs.
• Identifying individuals seeking services through the Department of Mental health database that could be a suitable fit for our agency. Assisting through the intake and transition processes.
• Being on the ON-CALL rotation (every other week).
• Monitor team performance and provide feedback to foster continuous improvement and professional development
• Ensure compliance with all relevant mental health regulations, confidentiality standards, and organizational policies.

Requirements:

• Assicates Degree Preferred in the field of human services (Social Work, Sociology, Psychology or related field) with 1 year experience working with individuals with developmental disabilities.
• Education can be substituted with proof of five years of continuous experience working with individuals with developmental disabilities.
• 1-3 years or more of supervisory experience.
• Travel: As needed
• Valid driver's license, acceptable driving record and current auto insurance.
• Social Security Card
• FCSR Background check and Drug screening
• Strong organizational and problem-solving skills.
• Willingness to take the trainings required by the Department of Mental Health.
• Has the ability to work independently but also a team player.
• Good judgment and the ability to remain professional at and outside of work.

Training Requirements:

All trainings are preferred but not required to be selected.

Training will be provided by Graystar, LLC.

• Level One Medication Aid (L1MA)
