What You'll Do
As a Home Care Coordinator, you'll serve as a key link between physicians, hospitals, patients, caregivers, and our clinical team to ensure a seamless care experience. In this role, you will:
Coordinate communication between providers, patients, and clinicians.
Support medication verification and plan of care coordination.
Assist with scheduling, care coordination, and other administrative support that helps our clinicians deliver exceptional care.
Collaborate with your team to provide the highest quality experience for every patient.
We’re looking for someone who:
Has a clinical background (Licensed COTA or PTA).
Enjoys organization, communication, and problem-solving.
Is detail-oriented and compassionate.

Experience:
2 years of medical office experience and/or direct patient care.
Ability to multi-task in a fast-paced environment.
Ability to work and communicate professionally with patients, medical partners, employees, and
supervisors.
Required:
Current Minnesota PTA/LPN/COTA license in good standing.
Knowledge of medical terminology
Good judgment and decision-making skills.
Strong attention to detail.
Technologically proficient and driven by accuracy.
Regular attendance and flexibility with scheduling necessary.
The minimum salary is $65K and the max salary is $67K.
