Summary/Objective

Working as a member of the clinical team, the clinician provides excellent, protocol-driven reproductive, gender-affirming and primary care services. The clinician actively works in conjunction with clinic team members to advance the clinical scorecard demonstrating clinical skills proficiency and respect for patients and co-workers with differences in backgrounds, lifestyles, viewpoints, and needs in areas such as ethnicity, age, race, gender, creed, and sexual orientation.

The Clinician-Abortion Services is responsible for providing safe and consistent medical and surgical care as related to surgical (SAB) and medication abortion (MAB), ensuring established medical protocols are followed and policies, procedures, and practices are in place to deliver exceptional patient-centered services.

Essential Functions

• Perform comprehensive and routine focused physical examinations, diagnose and treat, prescribe medications, order and interpret (as needed) laboratory tests, perform screenings and in-clinic procedures and educate patients on healthcare options in support of exceptional reproductive and complementary healthcare services. Utilize electronic medical records and associated devices and equipment efficiently to support clinical practice.
• Provide excellent patient-driven customer service to patients consistently. Seek to anticipate patient needs and pro-actively work to increase the satisfaction of the patient experience. The patient experience includes their first impression which can be affected by the clinician’s demeanor in the exam room, level of professionalism exhibited, clinical assessment and treatment and the collaborative efforts of the clinical team to meet and exceed the patient’s expectations.
• Understand fully and adhere strictly to PPGWNI medical protocols. Practice within scope of licensure. Use online and paper protocols as reference documents effectively. Actively increase knowledge of protocols through continued reading, studying and attendance at clinician phone calls and meetings and keeps up to date on new and revised protocols. Function as a resource and role model to clinical staff on professionalism and medical protocols, procedures and practices. Understand the follow-up process and how to effectively utilize the Follow-up Team. To ensure practice is within parameters of PPGWNI medical protocols and scope of practice, proactively seek out training. Provide for mentoring and coaching for licensed and unlicensed clinical staff.
• Maintain a high-level of productivity and efficiency based on PPGWNI’s established patient schedule as supported by clinic staffing. Help ensure that the clinical team is working together in a cooperative and coordinated manner to ensure positive clinic flow and deliver exceptional health care services while meeting established productivity standards. Be a champion and positive change agent for new or revised medical protocols and services.
• Travel to other health centers as required in support of job duties. Must have own personal transportation, valid and unencumbered State driver’s license and current vehicle insurance.
• Flexibility in hours required to support patient scheduling including working evenings and Saturdays (where offered).
• Performs after-hours triage and consultation on a rotating basis.
• Review and evaluate patient’s health history including obstetric, gynecologic, contraceptive, medical, surgical, family health history, and psycho-social backgrounds. Evaluation includes the assessment of the effectiveness of current contraceptive methods and identification of future needs based on patient’s lifestyle and plans. Learn and use information about how the needs of a diverse patient population can be sensitively met.

• Perform physical examinations with special emphasis on the reproductive system including breast and pelvic exam, Papanicolaou smear, bimanual and recto vaginal exam and other specialized procedures. Based on findings and utilization of prior health history, formulate diagnoses and treatment plans including those for sexually transmitted diseases and other minor gynecological problems, in accordance with medical protocols. May refer more complex cases to a specialist.
• Perform gender affirming care per the medical standard and guidelines including evaluation of appropriateness, medication management, lab evaluation, support letters for transition and, referral guidance.
• Primary care for limited and expanded services (Anemia, Asthma, Depression/anxiety, Diabetes, GERD, Headaches, Hypertension, Hypothyroidism, lipid disorders, smoking cessation), as outlined in chapter 18 of the MS&G’s, this includes limited and expanded services only.
• Perform pregnancy diagnosis and determines gestational dating using bimanual ultrasound exams (if trained and privileged), fundal height palpation and orders pregnancy tests. All pregnancy options must be equally presented to the patient. If the provider determines that the patient would benefit from additional counseling, internal or external referrals are to be made.
• Maintain health center efficiency and productivity standards; patient schedules reflect four (4) patients an hour. Schedule may be revised due to low census, staffing, training, or proctoring.
• Performs clinical research and peer clinical education and training duties as assigned.
• Supports productivity expectations and compliance standards of the agency.

• Perform in-office procedures within scope of practice and privileging in compliance with PPGWNI medical protocols. Completely understand confidentiality and HIPAA and the repercussions if breached. Actively work to maintain and protect patient confidentiality and not get involved in inappropriate conversations about patients.
• Assume legal responsibility for prescribing, ordering, and dispensing of drugs and supplies in compliance with State and Federal laws and in accordance with PPFA/PPGWNI medical policies and procedures.
• Provide information and counseling on family planning methods: oral contraceptives, IUDs, diaphragms, cervical caps, condoms, vaginal spermicide, sponges, natural family planning, ring, patch, injectables, etc. Order, dispense, fit or insert preferred method ensuring appropriate consents are obtained. Assess patient appropriateness for an IUD, implants, and manage insertion, follow-up care, patient instruction and removal.

• Provide information and education options in areas of health maintenance and reproductive care such as nutrition, infertility, sexuality, family planning, self-breast exam, menopause, HIV antibody testing, and safer sex techniques.
• Efficiently utilize the electronic medical record (EMR) to records all appropriate information in the patient’s medical record using approved format, medical terminology, abbreviations, etc.
• Accurately completes the Encounter plans and billing tab through the EMR system indicating medical services provided, primary diagnosis code, conditions treated in the visit, medication dispensed, lab tests performed, and the practitioner’s name
• Consult with the Chief Medical Officer or Associate Medical Director on non-routine clinical matters including questionable pathological or contraceptive status. Provide medical information support to health center nurses and/or other practitioners with patient questions and problems. May assist with phone triaging.
• Accurately and consistently pay attention to detail while processing labs, specimen handling, lab equipment, appropriate and legible chart documentation, and lab follow up; fully understand use of diagnosis codes and use them correctly. Work towards 100% accuracy on chart audits. Significant positive upward trend is noted on all subsequent audits as defined by work plans.
• Actively work to increase teamwork, including having good attendance, being punctual, reliable, and flexible with the ability to travel to other sites to fill in as needed. Work varying hours. Work to maintain a positive working relationship with the Health Center staff. Work to positively resolve conflict with co-workers, anticipate co-worker needs and works to meet those needs. Take initiative during down time to complete necessary tasks. Communicate positively and hold self and team members accountable. Work well with multiple personalities; models and sustain an attitude of respect for all individuals. Work to increase the team spirit of the clinic.

• Fully understand financial resources and funding sources available to patients and how to best utilize them. Understand the differences between labs (Safety Net, Bremerton, CDD) and non-project labs and is able to disseminate information to patients clearly and thoroughly. Use diagnosis codes appropriately, understand the importance of the clinician role in the clinic.
• Participate in annual skills checklist and bi-annual chart review, conducted by the Chief Medical Officer, or Associate Medical Director demonstrating continued competence in scope of duties.
• Perform colposcopy exam, if trained in this specialty and follows-up with patient as appropriate.
• May participate in medical and nursing students proctoring as scheduled. May also be involved with other projects as assigned by clinical management.
• Provides medication abortion (MAB) services for patients seeking elective pregnancy terminations and support for surgical abortion (SAB) services within defined scope of practice. Explains procedures thoroughly and educates patients on potential complications. Complies with all Washington State Department of Health Services abortion rules and regulations, federal rules and regulations, and Planned Parenthood Federation of America policies, procedures and medical standards and guidelines.
• Orders pre‐operative, intra‐operative, and post‐operative medication, including contraceptives. Supervise post‐operative care until patient is stable and/or discharged as defined by protocol to prevent complications. Ensures thorough follow up care is provided and the patient receives adequate information on how and where to seek continued care.
• Model a positive work ethic and demonstrate excellent organization, multi-tasking, verbal and written communication skills with a high tolerance for change and complexity. Must be able to provide nonjudgmental, supportive education that facilitates patient learning and supports patients in identifying their needs and interests. Works to maintain a positive working relationship with the Health Center staff.

• Efficiency/Productivity: Sees 4 patients/hr and/or positive progress towards 4/hr, actively participate in improving clinic flow.
• Financial: Fully understands financial resources available to patients (insurance, 19c, take charge) and how to best utilize them, understands project and non-project labs and is able to disseminate information to patients clearly and thoroughly. Uses diagnosis codes appropriately, understands the importance of the clinician role in the clinic.
• Annual Skills Checklist and bi-annual chart review, conducted by the Chief Medical Officer and other designated staff. Must demonstrate continued competence in scope of duties.
• Annual Performance Review: Conducted jointly by the Chief Medical Officer and Health Center Manager.
• Function as a medical professional role model to clinic staff.
• Serve as a role model for our In This Together (ITT) workplace values and service standards.
• Other duties as assigned.

Organization Structure and Interfaces

The clinician interfaces with patients, patient partners, medical and health care assistants, the Follow-up Team and the clinical management team on a daily basis. The clinician may also interact with physicians’ offices for lab and referral coordination. The clinician may be involved in the training of residents and medical students periodically and interact with some vendors. Clinicians participate in periodic clinician meetings and clinician phone conference calls and in other calls/meetings as scheduled. The clinician also interfaces with the Chief Medical Officer/Associate Medical Director who supervise clinical skills and act as consultants on protocol questions as related to patient care.

Required Education and Experience

2+ years of clinical experience. Valid Washington state RN and ARNP licensure is required. NP, PA-C or greater required with a family practice or women’s health specialty preferred. Prescriptive authority, ACLS, and CPR required. Proficiency in standard computer operating systems and standard office equipment, with strong attention to detail, and excellent written and verbal communication skills are required.

Preferred Education and Experience

EMR (Electronic Medical Record) proficiency and knowledge of diagnoses codes and microscope use strongly preferred. Bilingual (English/Spanish) preferred with a $20k sign-on bonus.

Supervisory Responsibility

This position does not supervise other staff.

Travel

Travel will be to affiliate sites and other venues as needed. A valid driver’s license, vehicle insurance, and access to reliable transportation is required.

Position Type/Expected Hours of Work

Full-time or part-time (sign-on bonus prorated for part-time applicants). Days and hours can fall Monday through Friday, 7:30 a.m. up to 8 p.m. or until last patient or agreed-upon schedule. This position may need to work long hours and weekends as needed.
