NorthPark Health and Rehab of Cascadia is a modern 54-bed all skilled facility with private suites and bathrooms and a very large rehab gym. Our team is dedicated to delivering personalized, high-quality care to patients in a supportive and empowering environment. As a newly established skilled nursing facility since 2021, we have focused on enhancing the well-being and independence of our residents through comprehensive therapy services and compassionate nursing care.

Overview

We are seeking an energetic and motivated Director of Rehabilitation to lead our therapy department, including physical therapy (PT), occupational therapy (OT), and speech-language pathology (SLP). This leadership role is vital in ensuring the delivery of exceptional rehabilitative services that promote recovery, independence, and quality of life for our residents. The ideal candidate will be passionate about fostering a collaborative team environment and driving continuous improvement in patient outcomes preferably with experience working in Skilled Nursing Facilities.

Duties

• Oversee the planning, implementation, and evaluation of all rehabilitation programs across PT, OT, and SLP disciplines
• Lead, mentor, and support therapy staff to ensure high standards of clinical excellence and compliance with regulatory requirements
• Develop individualized treatment plans aligned with residents’ goals and medical needs
• Collaborate with interdisciplinary teams including nursing, physicians, and case managers to coordinate care plans
• Monitor department performance metrics and implement strategies for quality improvement
• Ensure documentation accuracy, timely reporting, and adherence to all licensing and accreditation standards
• Promote a culture of continuous learning, professional development, and team engagement

Skills

• Proven leadership experience in a rehabilitation or healthcare setting with supervisory responsibilities
• Strong knowledge of rehabilitation therapies including PT, OT, and SLP practices and regulations
• Excellent communication skills to effectively collaborate with staff, residents, families, and external providers
• Ability to develop strategic plans that enhance service quality and operational efficiency
• Active licensure or certification in respective therapy disciplines (PT, OT, SLP) preferred but not mandatory; leadership or management certifications are a plus
• Passion for resident-centered care with a focus on improving functional outcomes and quality of life

Pay: $105,000.00 - $125,000.00 per year
