Park View Post Acute is the preferred provider of Post-Acute Rehab services in Santa Rosa, CA and we are seeking a full-time Physical Therapist to join our team. We have an excellent reputation within the community with a long history of success. This is an active skilled-nursing facility which offers a stable in-house therapy department with an abundance of opportunity to hone your skills as a clinician and develop and create the programs that will best serve your patients. Duties:

• Effectively screens/evaluates patients with a wide array of diagnoses and movement related disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Provides rehabilitative, skilled, and medically necessary treatment intervention to patients with physical functioning disorders and/or wound care per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, wound care, etc.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Exhibits knowledge and understanding of therapy practices, as well as strong writing skills required for the documentation of the care you provide.

Qualifications:

• Licensed Physical Therapist
• This position is open to PTs at all experience levels. New Grads Welcome!
• CPR certified (preferred)
