The QIDP is responsible for the whole person using clinical case management. They provide person-centered support and oversight in the development, implementation, and monitoring of individualized service plans for individuals with intellectual and developmental disabilities, ensuring compliance with federal and state regulations. Leverage expertise in behavioral health, program coordination, and interdisciplinary collaboration to promote independence, dignity, and quality of life for individuals served. This person will ensure all host home sites are meeting expectations for the person served.

2. Manage all responsibilities based on the agency's mission, using individual choice, and all appropriate policies, procedures, and regulations
3. Conduct oneself in a way that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts
Department/Program Development and Coordination (Delivery of Quality Service)
1. As a mandated reporter, ensure all allegations of abuse and neglect are reported as required.
2. Ensure compliance with all HIPAA and confidentiality requirements.
3. Complete all mandatory documentation accurately, honestly, and on time.
4. Need to complete all required training within 120 days of initial employment and stay in compliance with all the required annual training refreshers.
5. Spending quality time with each person on the caseload to develop trusting relationships and support with whole-person care. Provide competency-based training assessments/on-the-job training for all staff in the assigned program area. This training shall include, but is not limited to, procedures, CBTAs, observation of people served, and specific instructions for shift duties and responsibilities. Provides additional required information and training to support DSPs.
6. Provide guidance and training to direct support personnel to ensure individual programming, documentation, safety, rights, and confidentiality are being met during service delivery.
7. Coordinate health services based on individual needs and licensure requirements. Schedules for psychiatric appointments, discharge staffing's, and community appointments are kept, and appropriate documentation is forwarded to the appropriate nursing staff.
8. As the 24/7 QIDP responsible to coordinate Community Day Services based on individual needs and preferences. Monitor service delivery and Individual satisfaction with programming and offerings.
9. Coordinate leisure and recreation activities for individuals, attend outings, and help individuals achieve community outcomes.
10. Collect, report, and utilize program data assigned. Review the documentation completed by staff to ensure that any problems or concerns are addressed promptly.
11. Obtain stakeholder input (individuals, families, guardians, and other staff) on improving services through individual/group meetings.
12. Complete monthly site environmental audits per DHS regulations and provide support to providers and persons while in the home.
13. Complete assessments and documentation include, but are not limited to, risk assessments, sexuality assessments, functional assessments, behavior plans, etc.
14. Responsible for ensuring all records are uploaded and maintained in individual electronic files, EHR files, or any other electronic monitoring.
15. Responsible for meeting the HCBS Settings Rule and providing updated materials as evidence.
16. Monitor all programs and services included in the personal plan to ensure that goals and objectives are met and documentation is completed for the assisted caseload. This includes completing all paperwork in accordance with DHS regulations and monitoring the implementation of personal plans by others.
17. Complete site audits and provide interventions with Host home Providers.
18. Attend community events such as trips, outings, etc, with the individuals on the caseload.
