Position Purpose:
With this private, multidisciplinary practice, you have the opportunity to make an important impact on the lives of your patients. We are seeking an enthusiastic Nurse Practitioner with background in vascular care, who also has great clinical and interpersonal skills to join this outstanding team practicing throughout the Chicagoland area. This NP will join a growing Multi-Specialty practice with a focus in minimally invasive procedures. The nature of this position involves treating patients at skilled nursing facilities, assisted living centers, and outpatient offices. We are seeking a candidate that is able to provide comprehensive care to a complex patient population with multiple co-morbidities and vascular issues. Patient management includes consultation, implementation of appropriate plan of care, radiological interpretation, and assistance with office-based procedures. The ideal candidate should be familiar with both arterial and venous disease management. The candidate must be detailed oriented, able to function within a multidisciplinary team, and be willing to travel to see patients within the Chicagoland area. Our organization is highly digital, and we expect the ideal candidate to be tech-savvy and demonstrate a willingness to learn new IT systems as applied to healthcare and customer relationship management.
Responsibilities:
- The Nurse Practitioner coordinates appropriate, timely patient care to achieved optimal clinical outcomes individualized to the patient
- The Nurse Practitioner performs full evaluation of patients, including thorough assessment to vascular disease, and implements a plan of care. NP must obtain HPI, physical exam, develop differential diagnoses, and formulate an assessment and plan
- Review and assess diagnostics and medications
- Order and review labs, develop plan of care, collaborate with interdisciplinary team, communicate with physicians to ensure quality outcomes, and reduce hospital readmissions
- Pre-op screening and Post-op care management
- Identify acuity of patients, and be able to determine appropriate setting for any required procedures (inpatient vs. outpatient)
- Appropriate and timely documentation
- Wound Assessment
- Provide patient and/or family education
- Assist in procedures
Qualifications and Skills:
- Master of Science in Nursing (MSN) Required
- Advanced Practice Registered Nurse (APRN) (Required)
- Candidates must be able to work independently and possess strong interpersonal communication skills, both verbal and written
- Previous clinical experience with Adult vascular medicine and Geriatrics considered a plus
- Licensed and clean record with Illinois Department of Financial & Professional Regulation
- Competent computer skills including experience with EHR, E&M Coding, and proficient typing skills
- Ability to multitask, fast learner, learn and utilize documentation systems appropriately

- Familiarity with electronic medical charting

- Ability to maintain professionalism and friendliness in working with challenging patients
