Position Overview

LTHC Solutions is seeking a compassionate, clinically driven Physician (MD/DO) to join our growing long-term care practice. This position provides an exceptional opportunity to care for residents in skilled nursing, rehabilitation, and post-acute care facilities while utilizing innovative technology, including telemedicine, Remote Patient Monitoring (RPM), and AI-assisted clinical communication.

Our physicians work collaboratively with Nurse Practitioners, Physician Assistants, facility leadership, specialists, and interdisciplinary teams to deliver high-quality, patient-centered care focused on improving clinical outcomes, reducing avoidable hospitalizations, and enhancing the quality of life for residents.

Position Summary

The Physician is responsible for providing comprehensive medical care to residents in assigned long-term care and skilled nursing facilities. This includes performing admissions, routine regulatory visits, acute evaluations, post-hospital follow-up, chronic disease management, and collaboration with interdisciplinary teams to optimize resident outcomes.

Physicians will have the opportunity to practice both onsite and through telemedicine, allowing flexibility while providing timely access to care across multiple facilities.

Primary Responsibilities Clinical Care

• Perform initial comprehensive evaluations and admissions.
• Conduct routine regulatory physician visits.
• Evaluate and manage acute medical conditions.
• Provide post-hospital follow-up care.
• Manage chronic medical conditions.
• Develop individualized treatment plans.
• Review laboratory and diagnostic testing.
• Coordinate specialty referrals when appropriate.
• Participate in advance care planning and goals-of-care discussions.
• Collaborate with hospice and palliative care providers.

Telemedicine & Remote Care

• Provide telemedicine services for acute and follow-up visits.
• Review Remote Patient Monitoring (RPM) data.
• Respond to abnormal clinical alerts.
• Utilize PlatinumLine AI clinical communication tools.
• Collaborate with bedside nursing staff to provide timely interventions.

Quality & Clinical Excellence

• Deliver evidence-based medical care.
• Support CMS quality initiatives.
• Assist in reducing avoidable hospitalizations and emergency department visits.
• Promote resident safety and quality outcomes.
• Maintain timely and accurate clinical documentation.
• Participate in quality improvement initiatives and interdisciplinary meetings.

Qualifications Required

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
• Board Certified or Board Eligible in one of the following specialties:
• Internal Medicine
• Family Medicine
• Emergency Medicine
• Active, unrestricted medical license (Texas preferred or ability to obtain)
• Current DEA registration
• Excellent communication and clinical decision-making skills

Preferred

• Experience in Long-Term Care, Skilled Nursing, or Post-Acute Care
• Telemedicine experience
• Geriatric medicine experience
• Knowledge of CMS regulations and long-term care practices
• Experience utilizing PointClickCare, Gehrimed or other electronic health record systems

Pay: $180,000.00 - $300,000.00 per year
