The Health and Wellness Center is seeking an experienced Behavioral Health Provider (LMFT, LCSW, LPC, etc.) to deliver comprehensive, culturally responsive mental health services to students within a School-Based Health Center (SBHC). Working as part of an integrated care team, the provider offers short-term, evidence-based interventions that address students’ behavioral health needs and support academic success, social-emotional development, and overall well-being. This role emphasizes prevention, early identification, brief treatment, care coordination, and collaboration with medical providers, school personnel, families, and community partners.
We are seeking a provider who can work 24-40 hours per week ($28-$32), between 7:30 and 5pm. Occasional late evening and Saturday availability is preferred. Exact days and times are somewhat flexible based on your preferences.
The successful candidate will perform a broad range of responsibilities including, but not limited to:
· Prevention, identification, treatment of behavioral/mental health and substance use concerns in a Short-Term Behavioral Health (STBH) model in an evidence-based Primary Care Behavioral Health (PCBH) setting.
· Evidence-based, short-term individual/group counseling that is culturally relevant, goal oriented, and helps patients to develop skills and resiliency.
· Safety assessments/plans and crisis intervention services.
· Responding to validated behavioral health and social determinants of health needs assessments.
· Integrated care coordination with Physician Assistant and support staff.
· Collaboration with schools.
· Referrals to outside agencies for long term therapy/intervention services and other resources.
· Coding and charting in electronic medical record (Athena).
· Data collection, participation in and leadership of quality improvement initiatives.
· Aptitude to learn and utilize virtual care service delivery/platforms.
· Meeting safety standards and compliance with policies, procedures, regulatory practices.

· Pay differential for bilingual (English/Spanish) candidates: $1000 1st year/$1500 2nd year
Requirements:
· Master’s degree from an accredited graduate school of social work, psychology, or counseling
· Professional licensure (LCSW, LPC, LMFT, etc) OR working towards clinical licensure (for the right candidate(s)
· Minimum of two years direct clinical experience (one-year clinical internship for unlicensed candidates)
· Working knowledge of DSM-V and diagnostic criteria.
· A strong service orientation and the qualities of flexibility, attention to detail, and ability to collaborate with multidisciplinary team members are critical.

The Health and Wellness Centers at Centennial High and Lincoln Middle Schools are two of ~4,000 School Based Health Centers in the United States. The Centers are operated by Every Child Pediatrics, a Colorado based pediatric non-profit annually providing care for over 24,000 young people. Medical and behavioral/mental health services are provided in- person and virtually at two school sites in Fort Collins, CO: Centennial High and Lincoln Middle Schools with plans for expansion to other school locations.
