Additional Medication Certifications, Hoyer lift experience, specific disease-state care experience, etc., are highly desirable but not required.

Willingness to travel to clients up to a 30-minute radius (from your home) is required.

POSITION SUMMARY:
To support new clients in Southern Ohio requiring home healthcare services, Home Care Network, Inc. is seeking 2-3 experienced, passionate, and professional Home Health Aides (incl. HHA, STNA, CNA, PCA or DoDD DSP) to join our home health agency and provide daily care, skill development, health monitoring, and advocacy to and on behalf of clients across Brown County, Ohio and surrounding areas.

Requirements / Non-Negotiables

Minimum of 1 year paid in-home care or caregiving experience
-
Actively-certified DSPs, STNAs and CNAs w/ +1 year experience are highly desired (or HHA/PCA with 2+ years paid experience)

DSPs also require a valid DoDD DSP certificate within the past 12 months and familiarity with Ohio Board of DoDD rules.

Successful completion of all HCN skills tests and competency evaluations.
- Willingness and timely compliance to maintain all ongoing Training & Certification Requirements

Active CPR + FA certification (+ proof/documentation of both readily available)

Medication Administration Certification is highly preferred

18yo + active Driver's License / State ID
- Reliable transportation to all scheduled visits, within full reach of stated travel radius.

Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid).

Consent + Ability to pass a State & Federal background check (including fingerprints)

Willing and able to regularly perform all physical demands of home care (including lifting/carrying up to 30–50 lbs. occasionally).

Starting Pay Rate (Hourly): $15 - $17 per hour
Pay Rate based on location, certifications, experience, schedule and travel availability, work history, references, client needs, and overall fit.

A successful Home Healthcare Aide provides personal care services under the direction of the Registered Nurse or Therapist in their region. You will be assigned to specific clients by the Registered Nurse or other appropriate professionals and perform services for clients as necessary to maintain personal comfort and safety in their home.
ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY:
As a Home Health Aide, you will:

Deliver direct patient care and support, following the established plan of care.

Measure and record vital signs and other specified metrics as instructed.

Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail & skin care, shaving, dressing).

Perform household tasks essential to health care and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).

Assists patients in taking medications under the direction and supervision of the Registered Nurse. This includes opening a container or pill caddy & handing it to the patient, but does not include administration of the medications.

Identify and report any abnormal findings or changes in the patient's condition.

Help with therapy exercises under supervision of our therapists (e.g., range of motion exercises, ambulation assistance).

Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.

Demonstrate safe practice in the use of equipment, avoiding equipment use until orientation has been provided. Notifying supervisor(s) of educational needs immediately.

Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested and through approved channels in a very timely manner.
