Duties
- Assess patients’ physical conditions through detailed evaluations of mobility, strength, and functional abilities.
- Develop personalized *care plans* tailored to individual patient needs, incorporating evidence-based practices such as *trigger point therapy* and other physiotherapy techniques.
- Implement therapeutic interventions to improve movement, reduce pain, and restore function across various settings including *acute care*, *home & community care*, *outpatient clinics*, and *hospital environments*.
- Monitor patient progress regularly and adjust treatment strategies accordingly to ensure effective recovery outcomes.
- Educate patients and their families on exercises, injury prevention, and health maintenance strategies.
- Coordinate with healthcare teams on *discharge planning* to facilitate seamless transitions from hospital or clinic to home or community-based care.
- Document patient progress accurately using *EMR systems* while adhering to healthcare regulations and standards.
- Stay current with advancements in *physiotherapy*, *orthopedics*, *geriatrics*, and other relevant fields to provide the highest quality of care.
Skills
- Strong knowledge of *anatomy*, *physiology*, and *medical terminology* relevant to physical therapy practice.
- Experience with diverse patient populations including *pediatrics*, *geriatrics*, orthopedic cases, and those requiring *home & community care*.
- Proficiency in developing and managing comprehensive treatment plans that include discharge planning and specialized therapies such as trigger point therapy.
- Familiarity with various clinical settings such as *inpatient*, *outpatient*, *hospital*, *home health*, and clinics.
- Ability to utilize *EMR systems* efficiently for documentation and patient management.
- Excellent communication skills for patient education and interdisciplinary collaboration.
- Knowledge of treatment techniques specific to areas like *orthopedics* and managing patients with complex needs such as post-surgical recovery or chronic conditions.
