Cost is good. Deductibles are low. This is better than most other employers.

Everything is average. Just like any average tech employer would give. So on its right are the Google, FB etc.

The vs a card can be great and a pain. They want documents for everything.

I was an intern there, so I don't have a great idea about the health insurance.

The insurance used to be awesome, but by the time I left, it was pretty aligned with all major tech companies in the area--that is, tolerable. The carrier they have for their coverage was Premera Blue Cross when I left, and I had it out with them several times over things that never should have been an issue. The Benefits team at Amazon was pretty great about stepping in to assist, which was one thing I liked about the Benefits program--at least there was a team that owned it and would help if you weren't getting what you were promised.
