Increasing premiums (but this is likely the norm for all US companies). Both PPO and High-deductible options. There is a spousal surcharge when you spouse declines coverage through their employer.

Excellent coverage. Pricing is good for single coverage, but TOO expensive to add any one else to the plan. :(
