Covers one annual physical and some diagnostic labs. The plan seems to be changed a bit every few years.

The insurance was moderately priced, but a bit higher than I had anticipated, given working for a healthcare system. The care was good, although the only coverage "in network" was through Gundersen.. so it limited care options. I really enjoyed working with my PCP at Gundersen--but it was tough to get in with her (unless scheduled far in advance) due to her extensive patient base. I think Gundersen is a top notch facility, and I do miss working there from time to time.

Great overall coverage for both in network and out of network providers within the tri-state area of Wisconsin, Iowa, and Minnesota.
