With Kaiser Permanente, convenient one stop shop, appointments sometimes hard to get in short notice but it's not a company problem.

Teeth coverage and kids support is also provided.

If you have to live in the US and have employer-provided health care, it's about as good as it gets. Still has annoying complexity like deductibles, out-of-pocket-max, reimbursement from pre-tax Flexible Spending (you have to guess how much or you lose the money)
