I applied online. The process took 4 weeks. I interviewed at Amazon (San Jose, CA) in Mar 2015
Interview
Had 2 interviews. Applied directly on amazon.com careers website. Amazon.com schedules interviews for everyone who applies. Received am email from recruiter 1-2 weeks after application.
Whole process takes about 1 month, right from applying to getting the decision. Good thing about interning at Amazon is that the internship program has been designed to choose full time employees from interns. At the end of the internship you get the hiring decision from Amazon.com depending on the performance in internship.
Bound by NDA so cannot specify the exact questions. Though the interviews were based on data structures.

Question 1
The first question is LeetCode style algorithms question, and the second question gives a full stack repo (choice of Java, NodeJS, or Django) and asks to solve a backend issue which is causing a bug in the frontend. Unit tests must pass to pass the second question. You can run both backend/frontend indivdually or together
