I applied online. I interviewed at Sam's Club (Bloomington, MN) in Nov 2014
Interview
you must apply on the sam's club website (some stores have a kiosk but I would call ahead if you do it that way to make sure they have one). then you call them after 2-3 days if they have not contacted you and ask about your application with a manager. If interested they will try and set you up with an interview the same day you called or the next day. you will be given 2 interviews, one with a store manager and the other with the team lead of the department. the next day give them a call about the interviews. they will give you the job offer right then if they are interested.

Interview
Single inperson interview, very easy and extremly personal, wore business casual. Always wear clothes for the job you want in the future not for the one you are applying for. I wore what I saw managers wearing there.

Interview
Interview was conducted within minutes of applying on the luck of a manager reaching out to me. But normally you apply online, and then they call you back if they want you. (Call back will take from 2-5 weeks)
