I applied online. The process took 5 days. I interviewed at Tutor Doctor (Longmont, CO) in Jan 2016
Interview
Applied online, was contacted via email within two days to set up an interview with the operations coordinator, Leah. She's very nice, you meet her in a conference room in downtown Longmont CO to interview. It's a very cold room, if it's cold outside. They rent out that space because all tutoring is done at the client's home or other location. I know it seems sketchy at first, but it is a real business they are just trying to cut fixed costs. (I assume)

Very good, Simple questions about previous experience. The lady was very kind and I felt very comfortable. If you pass the phone interview this interview is not too difficult. It took place at a coffee shop so not very formal. overall I had a good experience with tutor doctor
