I applied online. The process took 4 weeks. I interviewed at UnitedHealth Group (San Antonio, TX) in Feb 2013
Interview
I applied online in late Nov. 2012. I was then contacted by a recruiter in early Feb. 2013 for the initial phone screen. I was told that by Friday I would know something then I was contacted on Wednesday for the second interview with a hiring manager. Initially, this was supposed to be an in-person interview, but it was short notice and the manager agreed to a phone interview, which lasted an hour. I thought it went well the interviewer and I seemed to have a great connection. I was not contacted by the deadline I was given it was roughly a week later. Overall, the process is lengthy and an offer is contingent upon a background screen and drug test completion.
Side Note: Do NOT take an update of your profile with the phrase "no longer being considered" as final because I thought that then I received a call. I was also a long-distance applicant.

There were not many difficult or unexpected questions...I tried to do my research and relate past experiences.
The process took 5 weeks. I interviewed at UnitedHealth Group (Atlanta, GA) in May 2015
Interview
I received a call from the Recruiter 20 minutes after applying for the position. I was asked basic screening questions and was sent a link for Hirevue, which was awkward for me since it was my first time using a webcam for interviewing purposes. The next day, the recruiter contacted me and said I should hear from the hiring manager within 1-2 weeks. I received a call from the Director based out of Texas 3 weeks later and scheduled a phone interview for the middle of the week. The interview went smooth and I was told a decision would be made within 2 weeks. 2 weeks later, I received an email to complete the self identification in order to remain as a candidate for the position. As I attempted to complete the self identification, I viewed my profile and the status changed to "No longer under consideration." At this point, I was confused and thought that I wasn't selected until I received a call from the Recruiter offering me the position.
