Sharecare’s home care offering is expanding beyond Medicare advantage to deliver targeted care management programs and transitional care for high-risk populations on behalf of payors, employers, and health systems. As the trusted home care partner for several of the largest payors in the U.S., CareLinx by Sharecare is currently available to over 2 million Medicare Advantage members as a supplemental benefit, with nearly half of those members in transitional care programs. While health plans and self-insured employers actively seek ways to increase member usage across their well-being offerings, most yield low sustained engagement. But a new program from Sharecare - Get Active - is designed to engage employees in physical activity by enabling “exercise by accident” through a convenient and gamified movement experience using the Meta Quest 2. In a pilot with Lennar in late 2022, our Get Active program yielded strong results for Associates, including delivering an engagement rate of 82% and a 280% increase in average weekly exercise minutes per person compared to pre-program among high-risk participants. Sharecare’s Medicare-reimbursable cardiac rehabilitation program, Ornish Lifestyle Medicine, is now available through a virtual delivery model.
