lack of continuity in management, lack of execution of strategy, lack of cohesive vision, very inconsistent personnel management, complete lack of performance based incentives at a mid manager/career level, lack of operational guidelines for such simple things as bid generation and scope review, absolutely no opportunities for cross discipline or other types of training, many senior staff just looking for a good exit opportunity, many managers are so frightened for their security that they are not making decisions for sustainable success, but simply for short term survival
