Advancement is very much based on who you know not what you know. Management thinks it knows what's going on, but scrambles to adapt when changes come from customer, even when changes are predictable and obvious. There is no internal communication, workers are constantly in the dark as to what is going on and it feels like the workers are always being blamed for anything that goes wrong, never management.
