Constant moving priorities. Shifting Silos Little training for Specific unique equipment. "Figure it out" was a common answer. Sr management reversals in focus and duties. Sr management isolated itself from rank and file to formulate "strategies" without concern for realities. Focus too much on blame, and not on problem solving. rampant CYA efforts.
