Orders come all the way from the top, and most of the time, largely don't make very much sense when it comes to day to day operations at bases, sorta like someone is looking at the big picture, but doesn't see the small details that need addressed.
Many employees are... well, lets just say this: It's much easier to run a motivated and hard worker into the ground than it is to motivate someone lazy. A significant amount of folks "forget" how to do certain things when doing something is inconvenient to them. As a result, there's a high turnover of "good people."
While a bunch of micromanagy sort of policies that are supposed to be all encompassing come down the chain, if you ask for specification there's a distinct silence from anyone above "middle" managment within the company.
