Too much hierarchy. Technical decisions are not delegated to technical people. Technical people often afraid to take actions on their own. Too many levels of control leading to inefective and expensive decisions. If flatten the structure, reduce the cost of reporting numbers that when aggregated mean nothing and established a more direct culture company could leap ahead. But if the process of tightened reporting without actual relevance continues would continue to be nice and slightly boring company.
