Senior management's fondness for reorganising / promoting / demoting staff with no clear objective or measurable outcomes has resulted in low staff morale and an appallingly high turnover. Clients have little confidence in senior management so do not place any larger (economically viable) packages of work, preferring to direct and manage staff themselves. This drives down salaries to below the market rate, resulting in a steady flow of good staff to work for clients.
