Also unkept promises from management. Managers don't give a damn about you if you don't cause problems. Unless you are a close friend of theirs. It's better to break something and fix it later than to prevent it, because you'll be praised more that way. Questionable design decisions that result in missed deadlines, but no one is held accountable. Chaos in high- and middle-level management. Decisions are made at the last minute, without consulting stakeholders. Global decisions that affect the whole company even if countries work differently.
