Poor to no documented process. No to poor process review. Status quo approach and mentality is pervasive. No empowerment. No risk management strategy, problems are documented as risks. Too much early focus on customization and too little "off the shelf" leads to failure to deliver on time. Little support. Little willingness to change. More of a don't think, do as you are told than a practice your craft environment. Cliquey environment, where bullying by some tenured people is tolerated. Communication is not effectively managed across platforms. Projects are sold based on the dream (off the shelf) implementation and not on the reality (we don't have the resources and/or plan to get it intricately customized in the time promised).
