The short list:
legacy code, bad security practices, constant 1am phone calls, the blame game, horrible management, no work life management, constant shuffling of priorities, projects and people. Long hours. No respect. Constant turnover. When you force out superstars constantly due to CEO looking for excuses for constant failure, you have a dead company walking. Divisions between groups. Nepotism and favoritism run rampant. If you don't speak Russian don't think about lasting more than a few months. So much potential here.
