No investment in technology. Most PCs are 5+ years old. They still run XP and Office 2003. The only reliable systems are the ones from the "green-screen" era and any web based tool crashes at least a couple times a week. Internal knowledge is not there to build or improve systems but will not spend money to fix it. Every project is done cheap instead of right. Lack of communication between various departments so everyone is always reacting to things other departments do. Focus on costs without looking at any long term impacts. The last few years have been cuts, offshoring and more cuts decisions made at executive level while front line has been left to "figure out" how to do more with less. Everything is reactive instead of proactive. Leadership is afraid to be the first to market or try something different than the industry
