1. Very old systems (think MS DOS/mainframe) which made everything more convoluted and time consuming. Seriously, you literally type bottom to top. 2. Inadequate training- Ive worked in claims for 15 years, I've worked for several of the countries largest carriers and I can honestly say this was the worst training I've ever experienced. Was told after an 4 week training on claim /policy basics and 1 week of systems that most of the training would be on the job. Everything had to be reviewed by the mentors, which would be fine if it didn't cause a delay in the adjudication and number of daily decisions, and subsequently viewed as lack of productivity by management. Even a simple email for employer forms would need sign-off and would take a few days to receive back. This would hold up a easy claim review for weeks.

Multiple work streams going on with no plan or strategy. Lack of dedicated diversity resources overlapping people. Company reactive on diversity issues. Those behaving badly are rewarded and allowed to tear others down.
