- The Lean Management team had little to (sometimes) no understanding of Lean Management principles - No structured approach to continuous improvement (Kaizen was just a buzzword) - Lack of knowledge about key Lean concepts like Value Stream Mapping, 5S, or Kanban - Meetings were unstructured and ineffective—no standard work practices in place - Resistance to change and no real commitment to waste reduction (Muda) - Team leader ignored data-driven decision-making, relying on gut feeling instead of actual process metrics
