Poor Tech Team communication, Poor Communication with Employees about Outages Poor to NO Communication between technical teams There's NO sharing of work that the server, network or voice team will be doing that is provided to other teams, they the IT dept does NOT work together, they do NOT share information and they do NOT provide information about upcoming work to those environments that could impact other depts or end users until something breaks, maybe they provide why.
