Corporate management trusting local and far overburdened management to keep the ship afloat. IT infrastructure built on a mentality of - if we purchase a competitor, let's add their infra to existing infra and if it doesn't work, throw it at ops and engineering and blame them for failing when they struggle to fit a square peg in a triangular hole- Headcount way too low, but would not BE too low if a proper infrastructure was in existence. The ridiculously high number of user account problems, from synchronization to magically disappearing accounts, virtual pcs that are built with ridiculously small storage and NO ability to add more because no one bothered to think ahead when the contract was written, ill conceived migration projects with more downs than ups. All of these issues and more are why we have such high turnover.
