Changing culture, high percentage of new management, recognition of failures are often delayed and/or diluted amidst the rapid-growth environment.

Upper management is combative, aggressive and short sighted often putting their own ideas in front of the engineering teams recommendations. Many of them use divide and conquer tactics to pit people against one another. Maybe the most toxic management culture I've ever been involved in. Often it is hard to even come up with a good reason to engage with them except for not wanting to get shouted at later.
