Big news as Washington state’s largest purchaser of healthcare joins Regence’s Episodes of Care program. Great news for the 363,000 state employees and retirees who will be able to benefit from a better coordinated experience and lower costs. Proud to be collaborating with Regence, Washington State Health Care Authority, and providers across the region who share the goal to make healthcare more affordable, more transparent and more efficient.

Medicare patients face multiple barriers after being discharged from the hospital, such as social needs, comorbidities, medication mismanagement, low care plan adherence, and many more. Our new Transition to Home solution provides hospitals and health systems the people, tools, and technology to coordinate the clinical, social and behavioral needs of patients following a hospital stay - reducing avoidable inpatient readmissions and unnecessary emergency department visits.
