I have a PPO high-deductible policy, so my coverage is exactly what I need for my family and me. Low co-pays for primary care visits ($0 annual physicals), $30 specialist co-pays and self-referrals. Prescriptions are generally low-cost for generics, but name brand scrips can be higher costs.
