- Assignments were dumped on staff with little to no instruction - Priorities shifted constantly, often changing within the same day - Communication was practically nonexistent - Mistakes became unavoidable because the environment thrived on confusion - Instead of guidance or structure, workers were left scrambling to guess what management actually wanted
