Pros

Flexible scheduling, incredible colleagues, emphasis on high-quality, evidence-based care, and the ability to provide patients with an in-house, multidisciplinary support system

Cons

As the company grows, the clinician experience is deteriorating rapidly. A once collaborative, team-focused environment now feels driven almost entirely by profitability and aggressive patient-volume initiatives. Some service lines see daily schedules that exceed ten patients per day, sometimes for hours without a break, and rely on cancellations just to keep the workload manageable. This volume is incompatible with consistent, high-quality outpatient care, yet critical clinician feedback is either overlooked or bypassed entirely. Many clinicians voice concerns to colleagues and direct managers, but discussions aren't held in larger meetings, reflecting a culture where open dialogue feels limited, ineffective, or unsafe as burnout intensifies. Seeing a high number of patients booked, yet never knowing how many will actually show, creates a daily work routine that is both frantic and rushed, yet monotonous and repetitive. Workflows remain tedious, and the tech stack continues to impede efficiency. Rather than addressing these foundational issues, leadership puts the onus on clinicians to be “more efficient” with tools that are fundamentally inefficient, while rapidly rolling out new high-volume initiatives based on limited data and little justification. Overall, organizational direction feels inconsistent and increasingly erratic. The underlying sentiment between clinicians is that of feeling unheard, unsupported, unprotected from burnout, and undervalued. It’s disappointing to see the company moving faster and further in this direction.

Thank you for taking the time to share such thoughtful feedback — and for the incredibly important care you provide to our patients every day. We’re glad to hear the positives you noted about our supportive RD team, strong benefits, and flexible remote model. That said, the concerns you raised around workload, scheduling pressures, and tools are very real, and we want to address them transparently.

I have only been at Oshi a short time, but I already appreciate the commitment these team players demonstrate every day. They are champions of the proven Oshi model of care, and they have a fervent desire to help our patients every day.

Cons

Mission driven and well-run

Pros

Working at Oshi is perfect for me on all dimensions I'm looking for in my work. The company is well-run and the team is great to work with: we punch far above our weight in our planning and metrics-driven decision making, and the team is smart and fun to work with. We are mission driven, and working on a clinical protocol that can fundamentally improve GI care, and i think healthcare in general. Being a fully remote company, I love the flexibility and work/life balance (I've never gotten to spend this much time with my kids) but we still get the job done

Cons

It's not easy being at an early stage company, and there's always a bit more we want to do that we can get done. It's a constant tension. It works for me and I wouldn't have it any other way for myself, but it's important to be realistic about how much effort has to go in.
