Toxic culture (in GCM business). Failure to respond to clients needs will ultimately lead to loss of market share, revenue, etc. Need to foster innovation as print volumes diminish. Senior management sheltered from hard truths by mid-management (I was an MD with RRD so am familiar with the "managing up" culture at RRD).

Now defunct. This company was sales-driven to a fault. Struggled mightily to keep up with changes in the marketplace because of entrenched interests determined to maintain the status quo.
