Largest downfall is the CEO. He is extremely smart and has a great business mind unfortunately who also has Narcissistic Personality Disorder (NPD). He is untouchable as the CEO and acts that way. He leads by fear. Most in the corporate building are paranoid daily wondering "am I next" to be fired for a fabricated justification. Company doesn't have any real figure head or department looking out for employees. The CEO is judge, jury, and executioner and if you question or go against the grain you are no longer "a team player" and fired. I personally witnessed blatant discrimination of gender and ethnicity to justify lower pay for both corporate and field employees. They make company corporate policies or "benefits" like 12 remote days a year, then either never approve or only do for special circumstances with 2 weeks notice (not in the written policy). They put tuition assistance in the employee handbook as a benefit and in 3 years never once have I seen it approved. They have several of things just as smoke and mirrors for recruiting and helping leadership sleep at night.
I personally heard the CEO brag multiple times about is sexual acts with coworker in his office and was proud of it. CEO also takes photos of female employees in compromised positions and disseminates to the "boys club" as a joke. In any other company that behavior would not be tolerated at any level.
The CEO and executive leadership (through the CEO) are constantly "moving the goal post". What is an expectation one day is different the next and sometime changes within the same day! The company is lead and decisions are made by emotions, and erratic at best. Ironically the CEO has his weekly staff meeting that only a select are invited to and he is constantly saying, "it isn't personal, its just business" however his behavior and actions are polar opposite. He has this meeting solely to hear the sound of his own voice and to pat himself on the back and make himself believe he is a leader.
Another negative is the lack of flexibility, the working hours are very rigid and don't allow flex time or remote work. Even though 99% of the work at the corporate office can be done remotely. This is driven by the lack of trust from leadership as it stems from the NPD of the CEO.
